Welcome Back

NC ROOTS is an online learning community designed to strengthen the capabilities and long-term sustainability of rural health care organizations across North Carolina Medicaid Regions 2 and 5.
Developed as part of North Carolina’s Rural Health Transformation Program, the NC ROOTS Learning Community provides rural health care leaders and teams with practical learning, tools, resources, and expert guidance to address the operational, workforce, financial, and care delivery challenges facing rural communities.

Rural health care organizations operate in environments shaped by workforce constraints, changing community needs, geographic barriers, limited resources, evolving technology, and increasing financial pressures.

NC ROOTS brings practical learning together in one place to help organizations build capabilities across critical areas including workforce and care team models, financial and operational performance, technology-enabled care delivery, organizational partnerships and affiliations, and long-term financial sustainability.

NC ROOTS is designed to help participants move beyond understanding a concept to applying it in practice. Throughout the learning community, participants can access:

  • Learning Tracks & Modules focused on key rural health care priorities
  • Tools & Resources designed to help translate learning into organizational action
  • Knowledge Checks & Assessments to reinforce key concepts and evaluate understanding
  • Expert Perspectives connecting learning to real-world operational and strategic challenges
  • On-Demand Learning that allows participants to access resources when and where they need them

There is no single model for rural health care delivery. Organizations differ in their services, workforce, size, geography, technology, reimbursement arrangements, resources, and the needs of the communities they serve.

NC ROOTS provides frameworks, examples, and practical resources that participants can adapt to their own organizational circumstances. The goal is to help rural health care organizations make informed decisions, strengthen organizational performance, expand and sustain access to care, and build the capabilities needed to respond to a changing rural health care environment.
